2.  Plans We Represent

Assurance Benefits is a licensed Third Party Marketing Organization (TPMO) representing multiple Medicare Advantage (Part C), Medicare Part D Prescription Drug Plan (PDP), and Medicare Supplement (Medigap) carriers in Arizona. We do NOT represent every carrier or every plan available in your area.

  • We will clearly disclose which carriers and plans we can offer you
  • We recommend you also consult Medicare.gov and your local SHIP to compare ALL available options
  • Plan availability varies by county and ZIP code
  • Benefits, premiums, and formularies may change annually on January 1st
  • Not all plans are available in all service areas

3.  Medicare Advantage (Part C) Disclosures

  • Medicare Advantage Plans are offered by private companies approved by Medicare
  • You must be enrolled in Medicare Parts A and B to join a Medicare Advantage Plan
  • Medicare Advantage Plans must cover all Medicare-covered services
  • Some plans may require referrals; provider networks may be limited
  • Out-of-pocket costs, benefits, and premiums vary by plan and change annually
  • Enrollment is limited to specific enrollment periods (AEP: October 15 — December 7; IEP; SEP with qualifying event)
  • CMS Star Ratings: Medicare evaluates plans using a 1 to 5 star rating system. Star ratings can change annually based on CMS evaluations. A plan’s past star rating does not guarantee its future star rating.
  • Leaving a Medicare Advantage Plan may result in gaps in coverage; we advise reviewing all options before disenrolling

4.  Medicare Part D Prescription Drug Plan Disclosures

  • Part D plans are offered by private companies approved by Medicare
  • Formularies (lists of covered drugs) may change; your specific drugs may not be covered or may require prior authorization
  • You must compare formularies annually during the Annual Enrollment Period (AEP: Oct 15 — Dec 7)
  • Low Income Subsidy (Extra Help) may reduce your costs — contact SSA at ssa.gov or 1-800-772-1213
  • Medicare Prescription Payment Plan (MPPP) allows you to spread Part D costs across the year — ask us for details

5.  Medicare Supplement (Medigap) Disclosures

  • Medicare Supplement insurance is sold by private companies to cover gaps in Original Medicare
  • Medigap plans are standardized (Plans A, B, D, G, K, L, M, N in most states); benefits are identical across insurers for the same plan letter
  • Open enrollment: 6-month window beginning the month you are 65+ AND enrolled in Medicare Part B
  • Outside open enrollment, medical underwriting may apply (insurers may deny coverage or charge higher premiums)
  • Medigap policies do NOT include prescription drug coverage; a separate Part D plan is needed

6.  Sales Call Recording and CMS Marketing Standards

  • All Medicare sales calls are recorded as required by CMS (42 CFR §422.2264 / §423.2264)
  • You may decline recording; in that case, the sale cannot be completed over the phone and we will arrange an alternative
  • We do not make unsolicited in-home sales visits without a prior appointment you scheduled
  • We do not approach you in common areas (hallways, parking lots, waiting rooms) for Medicare sales
  • We will not discuss or enroll you in a non-health plan benefit (food, OTC, transportation) without first discussing plan health benefits
  • We will not offer gifts valued over $15 as an inducement to enroll
  • Scope of Appointment (SOA): We document the products you agreed to discuss before our meeting
  • We must leave all marketing materials with you at the end of an appointment

7.  Compensation Disclosure

As required by CMS regulations (42 CFR §422.2274 / §423.2274), Assurance Benefits receives compensation from insurance carriers for Medicare plan enrollments. This compensation does not affect the premium you pay for your plan. Medicare Advantage and Part D premiums are set by CMS and the issuing carrier — agent compensation does not change your costs.

We are required to disclose the general nature of our compensation upon request. Please ask us if you have questions about how we are compensated for any specific plan recommendation.
